Antelope Valley Community Support Program

Full time position on our team to provide case management services to individuals with a severe and persistent mental illnesses. Responsibilities include: 

1. Maintaining a caseload of 20 individuals with multiple, serious, chronic behavioral health issues.

2. Assist with identifying resources, coordinating supports, advocating and transportation.

3. Proficient time management skills and the ability to work both independently and as a team. 

4.Assist individuals in achieving objectives, maximizing independence and community integration. Achieved through on site support, modeling and training.

5. Proficient record keeping and transportation in personal vehicle.    

Requirements:

Bachelor's Degree and 2 years of experience in an human service related field.  Must have the ability to work with a broad range of individuals. (gender, race/ethnicity, substance abuse, mental health, family members and other professional)
