West Holt Medical Services in Atkinson is seeking a licensed Mental Health Provider(LIMHP, LMHP, LICSW, LCSW, PhD, PsyD) that will provide behavioral health services within our Primary Care Clinic. This individual would work closely in conjunction with the primary care providers to develop an integrated behavioral health model for care.  The goal being to better serve our patients by improving communication between the prescribing providers and the behavioral health provider in order to form a more cohesive treatment plan with regular follow up.  Job duties include performing psychosocial assessments, developing treatment goals, and providing therapy to patients. Services would be provided across the lifespan.  **If candidate is provisionally licensed as a mental health practitioner, social worker, or psychologist in Nebraska, practice will be supervised in accordance with Nebraska state license requirements.
