Health Center Association of Nebraska is Nebraska’s Primary Care Association and is dedicated to expanding and enhancing access to quality, community-responsive care at Nebraska’s health centers. Through our collaborative approach, we bring together healthcare providers, community partners, government and legislature, and other stakeholders to address the root causes of health disparities and create lasting solutions. HCAN plays a crucial role in empowering and strengthening community health centers by serving as a supportive partner and advocate for positive change in community health.
Through our values, we are:
- Accountable to Outcomes: HCAN is intentionally goal-orientated and prioritizes measuring and evaluating the impact of its trainings, programs, and initiatives, ensuring that they lead to tangible improvements in health outcomes for the community.
- Trusted Thought Leaders: Through technical assistance and training opportunities such as conferences, webinars, and peer groups, HCAN serves as a respected, trusted thought leader on primary care and community health issues. HCAN engages in continuous learning through active listening to seek solutions for health centers. HCAN builds relationships with organizations at the regional and national level for healthcare excellence and equity.
- Health Equity Champions: HCAN is unequivocal about our beliefs and advocates for policies and practices that promote health equity and address social drivers of health. Recognizing the pervasive influence of social drivers of health, we work to address the root causes of inequity, including but not limited to socioeconomic status, race, ethnicity, gender identity, sexual orientation, disability status, and geographic location. HCAN guides operational transformation in care delivery through partnership and service expansion and works to increase access to care.
- Intentionally Creating Change: HCAN proactively seeks opportunities to drive positive change within the healthcare system. From growing partnerships to creating innovative apprenticeship models, HCAN spearheads initiatives that help create change in the health centers and the healthcare workforce.
