ACT Mental Health Worker

South Central Behavioral Services (SCBS) is seeking a Mental Health Worker for the Assertive Community Treatment (ACT) team.  The ACT team is a community-based, multidisciplinary team serving individuals with severe and persistent mental illness.  The team provides services in the communities of Hastings, Grand Island, Kearney and surrounding areas. Travel is required and company vehicle is provided. JOB SUMMARY

Provide direct service to adults with mental health conditions in a mobile and interdisciplinary clinical team.

RESPONSIBILITIES AND DUTIES

• Advocate for clients and support skills for independent living
• Support individuals to set goals and build skills
• Facilitate accessing appropriate housing
• Perform majority of services in the community rather than an office-based setting
• Communicating and collaborating with physical health providers and pharmacy
• Participating as a team in provision of care
• Provide coverage for rotating weekend/evening “On Call”
• Completing documentation in an electronic health record

QUALIFICATIONS AND SKILLS

• A Bachelor’s degree and/or experience in behavioral health care, specifically with persons with severe and persistent mental illness is required.
• Certified Medication Aide or be able to complete the training within 90 days of employment.
• Valid driver’s license
