SUMMARY DESCRIPTION OF WORK
Professionals at this level have excellent communication skills with clients, family members and community stakeholders. Specific project, program or office responsibilities may occur at this level. Decision-making, problem solving, and highly developed interpersonal skills are critical. Heartland Family Service is implementing a Certified Community Behavioral Health Clinic which is designed to provide a comprehensive range of mental health and substance use disorder services to vulnerable individuals.The Coordinator will lead the case coordination effort for the Certified Community Behavioral Health Clinic (CCBHC) including training HFS staff involved in care coordination and supervising CCBHC case coordinators. This person is active in the community, partners with other agencies, and looks for opportunities to help clients fill gaps in their care.
Work Schedule: Monday through Friday, 8:30 a.m. to 5:00 p.m.
