Job Description

 

Job Title:  Advanced Practice Registered Nurse

Reports To:  Psychiatrist

 

Areas of Responsibility:  Supports the goals, philosophy and objectives of Community Action Partnership of Western Nebraska (CAPWN) Community Health Center; works with the collaboration of Community Health Center physicians within the approved clinical protocols established.  This position provides psychiatric assessments and medication management services for Community Action Health Center patients.  Is respectful of the confidentiality of the patients, co-workers and the Community Health Center.

 

Major Duties:

  1. Provides primary health care to the patients of the Community Health Center.
    1. Obtains medical histories and perform physical examinations.
    2. Diagnoses and treats common health problems such as infections and minor injuries.
    3. Completes psychiatric assessments and formulates differential diagnoses based on clinical findings.
    4. Orders, conducts, and interprets pertinent laboratory and diagnostic procedures with appropriate follow through.
    5. Formulates, documents, and implements outcome-based treatment plans.
    6. Prescribes monitors and evaluates psychotropic and related medications.
    7. Conducts individual, group, and family psychotherapy, as needed.
    8. Orders and interprets diagnostic studies such as lab work and x-rays.
    9. Communicates test results and follow-up to client.
    10. Prescribes prescriptions for medications and other treatments.
    11. Provides well-child care, including screening and immunizations.
    12. Provides health maintenance care for adults, including annual physicals.
    13. Assesses real or potential health hazards and current health status, including analysis of health behavior related to lifestyle and culture.
    14. Devises, implements and evaluates plans of care utilizing sound clinical judgments based on assessment of the physical, psychological, emotional, societal and environmental needs of the patients.
  2. Provides family planning services.
  3. Promotes positive health behaviors and self-care skills through education and counseling.
  4. Collaborates with physicians and other health professionals as needed.
  5. Consults, refers and collaborates with other disciplines involved in the delivery of total patient care.
  6. Documents accurately all assessments, interventions, client/family responses, medication dispensed or prescribed, procedures, treatments and other data in medical record.
  7. Utilizes the resources of Community Health Center and the surrounding area community.
  8. Assists the physician in making hospital and nursing home rounds.
  9. Performs Emergency Room calls with physician backup on a scheduled rotation.
  10. Maintains a clean, safe and efficient clinic environment for clients, visitors and staff.
  11. Demonstrates evidence of continuing professional growth by involvement in peer review, the review of protocols and procedures and participate in quality improvement within the Community Health Center.
  12. Attends Community Health Center meetings and in-service offerings and participates in assigned committees.
  13. Maintain current Cardiac Pulmonary Resuscitation certification (CPR).
  14. Perform other duties as assigned.

 

Working Conditions:

  • Clinic environment 90%, 10% training, meetings, and community interaction.
  • Well-lit, temperature-controlled clinical setting, with standard medical and office equipment available. 
  • Work is frequently stressful and requires meeting tight deadlines.
  • Work in a highly volatile program in a state of change with many interruptions and interactions.
  • Must be able to work irregular hours, some weekends and evenings and out of town training.
  • Frequent exposure to communicable diseases, toxic substances, medicinal preparations and other conditions common to a medical environment.
  • The noise level in the work environment is above average.

 

Physical Requirements:

  • Varied activities including standing, walking, reaching, bending, lifting.
  • Requires full range of body motion including handling and lifting clients, manual and finger dexterity and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.  
  • Manual dexterity sufficient to operate office and medical equipment.
  • Must regularly talk, hear and have close vision ability to record and communicate appropriate documents and records.
  • Occasionally lifting and carrying up to 50 pounds.

 

Minimum Qualifications:

  1. Licensed to practice as a Nurse Practitioner in Nebraska.
  2. Graduation from accredited master’s level Nurse Practitioner program.
  3. Psy Clinical experience preferred.
  4. BLS/ACLS/ATLS.
  5. Maintain and have a valid driver’s license with good driving record and insured vehicle available at all times.
  6. Establish and maintain a good working relationship with the consumer and provider community.
  7. Bilingual in English and Spanish preferred.
