Emergency Community Support is designed to help ensure appropriate identification and provision of needed services for eligible persons disabled by severe and persistent mental illness and/or substance dependence and in a crisis situation.
IN THIS ROLE YOU WOULD:

Develop and track the progress of individualized treatment plans

Provide crisis management
-
Provide face to face service management and community-based case management

Adhere to program documentation requirements

Rely upon motivational interviewing and other evidence-based approaches
Job Requirements
EDUCATION, TRAINING, AND EXPERIENCE:
Demonstrated skills and competencies in treatment of individuals with a behavioral health diagnosis, demonstrated by at least one of the following:
- Two years of lived experience
- Two years’ direct care experience in a human service field
- Two years of training in a human service field
- Bachelor’s degree in psychology, sociology, or related human service field, is preferred
Other Salary Information
Starting pay 19.50/hr but may be higher based on experience and qualifications
