Job Description

Primary Function:

Incumbents work under the direction of a Registered Nurse (RN), Licensed Practical Nurse (LPN), or designee performing basic nursing care in accordance with the Douglas County Health Center (DCHC) established policies and procedures as well as state and federal laws and regulations.

Essential Functions:

  • Assist residents with hygiene needs and activities of daily living (ADL’s).
  • Assist residents with restorative nursing based on care plan.
  • Establish and maintain effective working relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public.
  • Comply with Civil Service policies and regulations, collective bargaining agreements, County policies, and laws to create a cooperative, safe, respectful and quality work environment.
  • Maintain workspace, resident room, and common space cleanliness.
  • Obtain/replenish supplies, and notify supervisor of equipment malfunction.
  • Provide superior customer service to residents, visitors and coworkers.
  • Encourage and support residents in maintaining or regaining independence, accommodate individual choices, and assist in maintaining personal belongings.
  • Document vital signs, intake/output, care/restorative nursing, and other nursing actions in a timely and accurate manner.
  • Notify nurse immediately of any physical or behavioral change in a resident (e.g. difficulty breathing, difficult to arouse, confusion, skin breakdown, cuts, bruising, bleeding, blister).
  • Report immediately to supervisor any suspected abuse, neglect, exploitation, and/or misappropriation of resident property.
  • Comply with all confidentiality and privacy requirements and regulations.
  • Assist with admissions, transfers and dismissals including required document completion.
  • Attend and actively participate in rounds, daily report, and assigned meetings.
  • Observe and adhere to all safety and emergency procedures reporting any unsafe conditions to supervisor.
  • Accompany residents to appointments on and offsite as well as planned facility activities.
  • Attend continuing education courses and seminars to maintain Nursing Assistant certificate in good standing.
  • Participate in quality assurance and performance improvement (QAPI) activities.
  • Answer the phone as needed, and assist residents in placing calls.
  • Report to work at assigned location with regular, consistent attendance.
  • Perform other duties as directed and assigned.

Job Requirements

Qualifications:

  • High school diploma or equivalent required.
  • Successful completion of a State certified 75-hour Nursing Assistant course required and must maintain in good standing.
  • Basic life support/cardiopulmonary resuscitation certification required within six (6) months of hire and must maintain throughout employment.
  • Must be at least 18 years of age.
  • Six (6) months experience in a healthcare setting preferred.
  • Experience with electronic medical records preferred.
  • Completion of a pre-employment criminal background check, post conditional-offer drug screen, physical assessment, and health review required.
  • Completion of an annual health review required.
