Therapist uses his/her abilities to provide intensive in-home/clinical out-patient family services, using a therapeutic model.* Provides in-home therapy and family services to include: attendance at initial team meeting, assessments, education concerning early care and development, assists in forming a family support network, home-based support services and ancillary family-focused services* Assists families in establishing a support network and provides coaching in parenting skills* Provides 24/7 crisis intervention for referred families including on-call rotation* Engages the child and family in services through various assessments, including the bio-psycho-social assessment, therapeutic activities geared to achieve goals.

  • * Assists families in establishing a support network by utilizing family, kinship, and community resources
  • * Provides 24/7 crisis intervention for referred families including on-call rotation
  • * Engages the child and family in services through various assessments, including the bio-psycho-social assessment, family conferencing (case planning), service delivery and day-to-day practices
  • * Facilitates and documents therapeutic groups, including family meetings, parenting, or multi-family groups
  • * Must utilize leadership skills with the staff team members.
  • * Documents all contacts with families and collaterals on agency database
  • * Completes reports for court and provides court testimony as required
  • * Liaison with DHHS, school personnel, and other community provider.
  • * Ensures clients’ rights are protected, explain SFCS’s privacy policy and provide copies of SFCS’s Notice of Privacy Practices and Family Preservation Brochure on clients’ rights
  • * Reports critical incidents to DCF verbally within 12 hours, written within 24 hours
  • * Reports critical and unusual incidents to Risk Management.
  • * Assists in monitoring the overall client satisfaction of the program
  • * Meets with their Supervisor weekly to review assigned cases
  • * Submits requested reports to Supervisor
  • * Provides services within licensure and accreditation standards. Adheres to all pertinent SFCS Standard Operating Procedures (SOP’s), personnel policies and federal and state statutes and contractual terms
  • * Has working knowledge of and adheres to the Family Preservation contractual requirements outlined by the DCF
  • * Is knowledgeable of and follows all safety procedures
  • The ideal caseload will be 4-5 families, spend an average of 4 hours per week with clients, a minimum of 2 weekly contacts, in compliance with the FCT model.
  • Ability to prioritize cases and organize multiple case demands for multiple clients
  • Serves as liaison with DHS, courts, schools, other providers, and community.
  • Provide thorough documentation including treatment plans, progress notes, monthly reports and assessments and submitted within the required deadlines reflecting language that is related to the model
  • Complete and maintain all program fidelity documents in a timely manner
  • Assists in monitoring the overall client satisfaction of the program.
  • Provides intervention for clients in crisis situations 24 hour on-call, 365 days
  • Completes reports for court and participates in court proceedings as needed.
  • Submits all required paperwork required for billing
  • Provides treatment within scope and practice of training and education
  • Ability to establish and maintain good community relations with other professionals
  • Ability to work in diverse home situations, tolerate odors, and environments which may require safety precautions
  • Provides services unique to each family that promotes the achievement of safety, permanency and well-being for the children, including evidence-based and trauma-informed in-home services.
  • Implement interventions to ensure any gaps related to the family’s basic needs are filled while helping the family also plan for long term sustainability.
  • Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms.
  • Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
  • Ensures clients’ rights are protected. Makes sure all clients have received their rights and maintains a signed copy in each file
  • Is knowledgeable of and follows all safety procedures

Job Requirements

Requirements:

  • Master’s degree in Social Work, Psychology, Marriage and Family Therapy or Counseling from an accredited college or university.
  • Licensed or provisionally licensed to practice in the State of Nebraska
  • Must be 21 years of age
  • Must pass a drug screen, Nebraska State Patrol Criminal Background Check, DHHS APS and CPS Central Registry Checks, and Nebraska Sex Offender Registry.
  • Any other background checks required by the contract.
  • Lifting requirement of 30 – 50 pounds
  • Must have valid driver’s license, acceptable driving record and reliable
