Job Description

The Position

South Central Behavioral Services (SCBS) currently has an opening for a part-time LPN or RN Care Coordinator in our CCBHC program.  As LPN/ RN Care Coordinator will serve as the nurse care coordinator for people in our programs by providing professional psychiatric nursing services and care coordination to individuals with complex conditions (physical and behavioral health needs). The ideal candidate will be responsible for the care and treatment of these individuals in accordance with accepted nursing and medical standards as well as work closely with program staff to attend to the whole health needs of our clients. You would participate in treatment team meetings to discuss and address the ongoing needs of the clients and are also responsible for completing daily documentation in our electronic health record.  The starting hourly range is contingent on experience and education, with additional bonuses based on productivity and performance. A hiring bonus is available after 6 months of successful employment.  Graduation from an accredited school of nursing and licensed to practice in the State of Nebraska is required; one year of experience in medical/surgical nursing with an additional year of experience in psychiatric nursing preferred. Job Requirements
